提交費用申報

Timely filing has past for all States that Bright Healthcare served. If you believe you have a claim that is still valid, you must provide documentation to support the processing/reprocessing of the claim past timely filing. Follow the directions below to submit your claim.

Provider's W-9 are necessary moving forward for any new payments as we process/re-process claims decisions due to appeals, provider disputes, and/or grievances. You MUST submit the provider's W-9 to receive payment, if appropriate.

Availity.com

(ONLY: CA, GA, TX, UT, and VA. Other states must use manual processes outlined below.)

接收付款

自2022年1月1日起，Bright HealthCare使用InstaMed處理AZ、CO、FL、IL、NY等州的聯邦醫療保險優惠計劃，以及CA、GA、TX、UT、VA等州的商業IFP的申報費用的付款。 如果您先前已經註冊登記InstaMed，則Bright HealthCare已預先設定向您發送電子轉帳。

Bright Health will execute commercial claim payments internally for the States of AL, AZ, CO, FL, IL, OK, NC, NE, SC, and TN. Payment will come in the form of a check.

InstaMed

• 請登入Availity檢視您的EOP。也可在InstaMed註冊登記以在網上使用InstaMed；InstaMed是一個免費的服務提供者入口網站，安全可靠，您可在此全天候隨時查詢付款詳情，並且列印匯款資料。
• 您也可選擇將EOP傳送到您現有的資料交換中心。

• Your EOP will be mailed separately from your check. The same claim # will be printed on both the EOP and the check so you may align your documentation.
